Health Forms(Required):
These are the State of Connecticut Department of Education Childhood Health Assessment forms. They need to be filled out and updated in alignment with State regulations.
Media Release(Required):
Our Lady of Fatima School and the Diocese of Bridgeport have active marketing programs. Students and their work may appear in the school newsletters, newspapers, brochures, videos, websites and official social media pages as well as other promotional material. Such productions may be used for educational or marketing purposes and may be copied or copyrighted with the school retaining any and all rights. Parents and guardians must give or deny consent in regards to the use of a child’s image by completing this form.
User Agreement for Electronic Information and Resources(Required):
Each student who utilizes the school’s computer system and other school electronic devices will be instructed in the proper use of the network, and the student and his/her parent or guardian must read and sign this use agreement before the student will be allowed to use the school’s computer system and electronic information resources.
Parent Code of Conduct:
If you are a new family enrolling at Our Lady of Fatima School, Wilton and would like to be considered for tuition assistance, please complete the Grant and Aid application at www.FACTSmgt.com/aid and submit required supporting documentation.
After School Letter and Agreement :
The program provides a structured, safe and supervised place to be after school for fun and friendship with adults and schoolmates. The program helps children to develop social skills and learn to handle conflicts in socially acceptable ways. Children who participate in the After-School program must follow the program rules and regulations.

Athletics Department:
2-Hour Virtus Training:
All our volunteers are required to be Virtus trained. Please visit the Virtus website and select "Bridgeport, CT (Diocese) from the dropdown menu. You will find a schedule of upcoming trainings in our area. Training sessions generally take 2 hours.
If you need to renew your training (after five years), please contact Diane Favier, Fatima School Administrative Assistant at dfavier@olfcatholic.org.
